Crimes or acts of public corruption committed by or against the County and City of Dallas, Texas, its employees, trustees and elected officials can be reported at any time using the form below. Many acts of public corruption are discovered through tips from honest people who are not sure of all the facts. Even when you have no proof, you can still provide a description of your concerns and why you believe a crime or act of corruption has occurred. All reports are treated seriously.
 

1. What has happened?  Please be as specific as possible.

2. Who is involved?  Please include the name, approximate age, and any other identifying information about the persons involved.
3. When and where did this happen?  Please provide the approximate dates and locations that the suspected crime or act of public corruption occurred.
4. How much money is involved?
5. Are there any witnesses?  If so, please provide their name, approximate age, and any other identifying information available.
6. Is there any evidence? If so, please be as specific as possible to include a description of any physical evidence and its location.
7. What do you think should be done? Please provide any suggestions that you may have for investigating this matter.
